What is on the NREMT EMT exam?
To earn National Registry EMT certification you pass a cognitive (computer) exam and a psychomotor skills exam. Since 7 April 2025 the EMT cognitive exam follows a new test plan based on the 2023 practice analysis. The five old areas (airway, cardiology, trauma, medical/OB/GYN, operations) were replaced by five domains that follow a patient call from start to finish:
- Scene size-up and safety (15-19%): hazards, PPE, number of patients, triage and resources.
- Primary assessment (39-43%): general impression, responsiveness, airway, breathing, circulation and life threats.
- Secondary assessment (5-9%): focused exam, history and reassessment.
- Patient treatment and transport (20-24%): airway and ventilation, circulation, medications, special populations and transport.
- Operations (10-14%): equipment, documentation and responder well-being.

Source: nremt.org (EMT candidate handbook and the updated EMR and EMT examinations page), checked 17 October 2026. Because the exam is adaptive, it stops as soon as the computer is confident you are above or below the passing standard, so a short exam does not mean you failed. The new exam also has technology-enhanced items such as build-list and drag-and-drop; MyLevel practises the knowledge behind them with single-answer questions. If you fail, you wait 15 days before retesting, and you have six attempts (remedial education is required after three).

How to pass the NREMT
- Put your study time where the points are: primary assessment is about 40% of the exam. Ask yourself on every question: what kills this patient first?
- Think like an EMT on scene: safety first, then life threats, then history and the focused exam. Many wrong answers are good actions done at the wrong moment.
- Know the numbers: compressions 100-120 per minute, adult depth 2-2.4 inches, 30:2 (15:2 for two rescuers on a child or infant), 1 breath every 6 seconds for an adult with a pulse.
- Stay in your scope: EMTs give or assist with a short list of medications (oxygen, aspirin, oral glucose, epinephrine auto-injector, naloxone, and the patient's own nitroglycerin or inhaler) by protocol.
- Practise under time: take a full test in one sitting, then study the explanation of every mistake and retake the domain you scored lowest on.

Sample question with explanation
Negligence requires duty to act, breach of that duty and causation. Which fourth element must also be present?
- A written complaint from the patient's family
- Intent to harm the patient
- Abandonment of the patient at the scene
- Damages: the patient suffered harm
Show the answer
Answer: D. Damages: the patient suffered harm
Negligence needs all four: a duty to act, a breach of that duty, harm (damages) and proof that the breach caused the harm. No intent is required.
Frequently asked questions
Is this NREMT practice test free?
Yes, you can start with 5 free questions with explanations, one from each domain. Two full 70-question timed tests cost $9.99 once, with no subscription.
How many questions are on the NREMT EMT exam?
The EMT cognitive exam has between 70 and 120 questions, including 10 unscored pilot questions, with a 2-hour limit. It is computer adaptive, so it ends when the computer is confident whether you are above or below the passing standard (nremt.org).
What score do you need to pass the NREMT?
Results are reported on a scale from 100 to 1500 and the passing point is 950. There is no fixed percentage, because the adaptive exam measures your ability level rather than counting correct answers. On MyLevel, aim for at least 75% and 85% for a safe margin.
Is the NREMT exam hard?
It is meant to check entry-level competency, and many questions ask what you should do first, so several answers can look right. Knowing the current domains, practising under time and learning from explanations help most.
Are these the real NREMT questions?
No. These are original practice questions written by MyLevel on the published content domains. Real exam questions are confidential. Our questions are single-answer multiple choice; the real exam also has some technology-enhanced items.
Can I unlock only this test?
There is one option, so you never have to choose: the $9.99 job pack, one time. It unlocks the 2 full tests on this page and every other MyLevel job test, with no subscription and a full refund within 14 days if it does not help you.
Do I get a certificate?
Yes. After a full practice test you can download a MyLevel practice certificate (PDF) with your score. It is a personal record of your practice result, not an official document or EMT certification.
