How many questions are on the NCLEX-RN?
The NCLEX-RN is a computerized adaptive test (CAT): every candidate gets between 85 and 150 items, with up to 5 hours including breaks. 15 of the items are unscored pretest items that you cannot recognize. The computer picks each next item based on your answers so far, so two candidates rarely see the same exam.
| Client Needs category (2026 test plan) | Share of the NCLEX-RN | Per MyLevel test |
|---|---|---|
| Management of Care | 15-21% | 14 |
| Safety and Infection Prevention and Control | 10-16% | 10 |
| Health Promotion and Maintenance + Psychosocial Integrity | 6-12% each | 13 |
| Pharmacological and Parenteral Therapies | 13-19% | 12 |
| Basic Care and Comfort, Reduction of Risk Potential, Physiological Adaptation | 6-12%, 9-15%, 11-17% | 26 |
| Total | 75 |

What is on the NCLEX? The Client Needs categories
- Safe and Effective Care Environment: management of care (priorities, delegation, consent, advance directives, confidentiality, ethics) and safety and infection prevention and control (isolation types, PPE, falls, restraints, fire and equipment safety).
- Health Promotion and Maintenance: growth and development, pregnancy, birth and newborn care, aging, screening and prevention.
- Psychosocial Integrity: therapeutic communication, crisis and abuse, suicide risk, substance use and withdrawal, mental health conditions, grief.
- Physiological Integrity: basic care and comfort, pharmacological and parenteral therapies (including dosage calculation and blood products), reduction of risk potential (lab values, complications) and physiological adaptation (acute and life-threatening conditions).
The real exam uses several item formats: multiple choice, multiple response (select all that apply), and Next Generation NCLEX (NGN) clinical judgment items, including case studies of six items each. The MyLevel tests use single-answer questions only, so practise the other formats as well. All questions here are original practice questions written from the public test plan, not real NCLEX questions.
How the NCLEX is scored: pass or fail
The NCLEX gives a pass or fail result, not a percentage. After the minimum number of items, testing stops as soon as the computer is 95% certain that your ability is clearly above or below the passing standard. If your ability is very close to the standard, you keep going to the maximum length and the final ability estimate decides. If time runs out before you answer the minimum number of items, the result is a fail. The passing standard is set by the NCSBN Board of Directors and reviewed every three years.
The NCLEX registration fee in the US is $200; NCSBN has announced it will be $350 from 1 February 2027 (nclex.com, fees and payment, checked 17 October 2026). Your state board of nursing may charge its own licensure fee.
Because the real exam is adaptive, no practice test can predict a pass. MyLevel shows your score per section and an indicative range so you know where to study.
How to use these NCLEX practice questions
Start with the 5 free questions and read every rationale. Then take one full test under time, look at your weakest section, study it, and take the second test a week later.
- Prioritization: airway, breathing, circulation; acute before chronic; unstable before stable.
- Delegation: the RN keeps assessment, teaching and evaluation.
- Lab values and ABGs: potassium 3.5-5.0 mEq/L, INR 2-3 on warfarin for most indications, pH 7.35-7.45.
- Pharmacology: antidotes, insulin types, what to check before giving a drug, high-risk IV rules.
The full pack costs $9.99, one time: two timed 75-question tests weighted by the 2026 test plan, a rationale for every answer, your score per section and a practice certificate. No subscription and no time limit on access. One purchase also unlocks every other MyLevel job test.
MyLevel is independent and not affiliated with, or endorsed by, the National Council of State Boards of Nursing (NCSBN). NCLEX and NCLEX-RN are registered trademarks of NCSBN. All questions are original practice questions based on the public NCLEX-RN test plan, not real exam questions.
Sample question with explanation
A conscious client with diabetes has a blood glucose of 58 mg/dL and can swallow safely. What is the nurse's first action?
- Give glucagon by injection at once
- Recheck the blood glucose in 1 hour
- Give the scheduled insulin dose with lunch
- Give 15 g of fast-acting carbohydrate
Show the answer
Answer: D. Give 15 g of fast-acting carbohydrate
For hypoglycemia in an alert client who can swallow, give 15 g of fast-acting carbohydrate and recheck the glucose after 15 minutes (the rule of 15). Glucagon is for a client who cannot take anything by mouth, and insulin would lower the glucose further.
Frequently asked questions
How many questions are on the NCLEX-RN?
Between 85 and 150 items, with a time limit of 5 hours including breaks. 15 items are unscored pretest items. The exam is adaptive, so it stops as soon as the computer can make a pass or fail decision (NCSBN 2026 test plan).
What score do you need to pass the NCLEX?
There is no percentage score. The NCLEX is pass or fail: the computer compares your ability with the passing standard set by NCSBN and stops when it is 95% certain you are above or below it.
Does the NCLEX have select-all-that-apply and NGN questions?
Yes. The real exam has multiple-choice, multiple-response (select all that apply) and Next Generation NCLEX clinical judgment items, including case studies. MyLevel's practice tests use single-answer questions only.
Are these real NCLEX questions?
No. These are original NCLEX-style questions written by MyLevel on the topics in NCSBN's public test plan. Real NCLEX questions are not published, and no site can legally offer them.
What do I get for $9.99?
Two timed practice tests of 75 questions, weighted by the 2026 Client Needs categories, a rationale for every answer, your score per section with an indicative range, unlimited retakes and a practice certificate. One-time payment, no subscription, and it also unlocks every other MyLevel job test.
Can I unlock only this test?
There is one option, so you never have to choose: the $9.99 job pack, one time. It unlocks the 2 full tests on this page and every other MyLevel job test, with no subscription and a full refund within 14 days if it does not help you.
Do I get a certificate?
Yes. After a full practice test you can download a MyLevel practice certificate (PDF) with your score. It is a personal record of your practice, not an NCLEX result, a nursing license or an official document.
